What you'll be responsible for:
* Remote assessment and treatment of patients with COPD, asthma, and comorbid conditions including congestive heart failure
* Clinical leadership of a team consisting of health coaches, respiratory therapists, and RNs
* Communicate with patients to understand their case and provide education to help improve outcomes
* Maintain high patient satisfaction scores
* Maintain timely accurate entry of patient interactions in the EMR (Athena)
* Supervise the performance and review of remote spirometry
* Participate in continuous process improvement and contribute to innovation
This may be a fit if you have:
* A license as a Nurse Practitioner in one or more states, with a background in respiratory care- CO being main focus area (MA, MO, MI, TX, UT, CA, IN, KY, WI-- additional states of focus for licensing)
* Both pediatric and adult experience is strongly preferred
* Outstanding communication skills, including the ability to be personable and supportive of patients
* Experience providing telemedicine care to patients
* Proficient computer and internet navigation skills
* Passion for innovation and process improvement
* Experience leading, educating, and mentoring non-licensed team members
* Looking to work 20-30 hours/week which could be flexible, such as 4 hours/day x5 or 8 hours/day x 3 (1099)
